step 1: Click on the order from that is on top of the home page then print the order form.
step 2: Fill out the information, design what kind of gold teeth you want and pay with credit card or money order.
*A. credit card : Click on the Quick Payment that is on the top of the home page. pay the total price and we will send you the molding kit. Then send us the mold with the completed order form. If you already have a mold you can send it with the order form.
*B. Money order : Send us the money order with the order form and then we will send you back the molding kit. After you make the mold send it to us.
